User Authentication Help

For your security and the privacy of your data, the Motor Vehicle Commission requires that you authenticate by entering the following information that corresponds to the record you are adding emergency contact for:

Enter current Zip Code
Enter current Driver's License Number/Id Number
Enter full Social Security Number (No spaces or hyphens)

As an extra measure of security, please enter the text you see in the image displayed into the entry box below the image. The image may contain numbers, upper-case letters and lower-case letters. You will have a limited number of attempts before the system will require you to log in again.

Add/Edit Next-Of-Kin Registry Help

Up to two emergency contacts can be entered. If you already have an emergency contact on file, it will be displayed. If you wish to change it, you can do so by typing over the current contact information displayed and select Submit to save the contact information.

Enter a Title (optional), First Name, Middle Initial (optional), Last Name, Primary Phone Number, Alternate Phone Number (optional). Select Submit to save contact information.

First Name, Last Name and Primary Phone Number are required. Phone numbers should be entered with area code, followed by exchange and number. Phone numbers are required to be 10 digits.

Please confirm that the emergency contact information that is displayed is correct. If you need to make changes, select Edit Contact(s) and make the appropriate changes by typing over the information displayed.
